A diagnosis for mesothelioma is a long process that can take months. Since this type of cancer is so rare and has such a poor prognosis, other possibilities are ruled out first and mesothelioma isn't a diagnosis that is jumped to quickly. It is not uncommon to be misdiagnosed after the appearance of the early symptoms because more often than not, those particular symptoms are caused by a completely different ailment.

 The tests for mesothelioma start out with the least invasive. Chest x-rays can be done to find any fluid build up that may indicate mesothelioma or other respiratory issues. If a tumor is suspected, a CT scan may be done instead because this type of imaging can detect tumors a bit more easily. Once a tumor is found, a PET scan is a logical next step. This type of scan involves an injection of a non-toxic dose of a radioactive substance that will show up on the scanner. The resulting images  can help determine whether a tumor is benign or malignant.  In order to see if the cancer has spread to other parts of the body, an MRI may be needed.

Before or during the process of scan, blood test can be taken to help determine if mesothelioma is present. The objective of the blood test is to find evidence of increased mesothelin in the blood stream. Mesothelin is a type of glycoprotein that is produced in excess amounts by the cells in mesotheliomas. A by-product of mesothelin, called N-ERC, can also be detected in a blood test and indicate the presence of mestheliomas. Although the accuracy is debated, another glycoprotein call fibulin-3 is marker of mesotheliomas that may appear in a blood test. Due to the research of fibulin-3 being still on going, it is currently not used often for an initial diagnosis but may actually be better as a tool to help determine whether the extent of the cancer.

​If the scan and blood tests point to the presence of mesotheliomas, the next step in determining a diagnosis is a biopsy. The goal of a biopsy is to get either a tissue or fluid sample that can be examined to find out if the cells are cancerous.  Since fluid build-up is very common with all types of mesothelioma, the main form of biopsy is extracting and examining the fluid. This is done by inserting a needle into the area where the fluid has built up. When done to extract fluid from the pleura, this procedure is called thoracentesis. If fluid has to be extracted from the abdomen, it's called paracentesis. When aimed towards the pericardium, it's referred to as pericardiocentesis. Despite the different names, the procedures are essentially the same and are just aimed towards different locations. Other mildly invasive procedures to obtain a sample for biopsy are thoracoscopy (pleura), laparoscopy (peritoneum), and mediastinoscopy (pericardium). Those involve using a small scope with a camera that is capable of not only seeing into the targeted area but also taking a small tissue sample.

The most invasive method of determining a mesothelioma diagnosis is surgery. This is used when the results of the other methods aren't conclusive enough or the tumor is in a spot that can't be effectively reached by less invasive methods. For mesotheliomas in the pleura and the pericardium, an open chest surgery called a thoracotomy may be performed. If the mesothelioma has formed in the peritoneum, it is reached through an abdominal surgery known as a laparotomy. Although primarily used for biopsy, the surgeon may immediately remove any tumors found as part of the surgery.
