Surgery is most effective for patients with the earliest stages of mesothelioma. When the cancer is localized to a single area, it can be removed much easier and there's only a small chance of the cancer spreading in the aftermath of the procedure. The majority of surgeries are preformed for patients whose diagnosis falls within the Stage I to Stage III range, although surgery becomes progressively more difficult as the cancer develops.

For pleural mesothelioma, there are three surgical options: Extrapleural pneumectomy (EPP), pleurectomy/decortication (P/D), and debulking. These surgeries differ in how extensive they are but the goal of all three is to remove the pleural lining and the surrounding structures that have been affected by the cancer or are at immediately risk of being affected. Debulking is also useful for peritoneal mesothelioma and aims to remove the as much of the affected peritoneum as possible. A second surgical option for peritoneal mesothelioma is an omentectomy. In the case of testicular mesothelioma, a radial orchiectomy is the only type of surgical option that has been shown to be effective. During this surgery, the affected testicle is removed along the spermatic cord.
Regardless of whether surgery is an option or not, chemotherapy and radiation therapy are often a part of the treatment regiment. Mesothelioma is more resistant to chemo and radiation than most types of cancers and these treatments are highly unlikely to result in the complete removal of the cancer but they have been shown effective at shrinking the tumors and slowing down the progression of the cancer.
Fluid build-up is a common symptom of mesothelioma and there are procedures that can eliminate the complications and pain that results from it. A thoracentesis (for pleural mesothelioma), paracentesis (for peritoneal mesothelioma), or a pericardicentesis (for pericardial mesothelioma) consists of a needle being inserted to drain the excess fluid. These are relatively minor procedures that are used only for symptom relief and as a form of biopsy.
If the mesothelioma is slowly progressing, postponing treatment and simply monitoring the disease is an option. Treatments can be hard on the body with a lot of side effects so if the cancer is growing slowly enough that it's not at risk of immediately spreading, waiting until symptoms appear to start treatment can allow for longer duration of comfort.
