When talking about cancers, a term 'stage' is used to refer to the extent at which the cancer has progressed. This classification system is essential to determining both a prognosis and the most effective course of treatment. Different cancer types may use staging classification systems with slight variations to fit the specifics of that particular cancer but they all have the same basis.

Due to being so rare, neither peritoneal nor pericardial mesothelioma have an official staging classification system like pleural mesothelioma does. However, they are all staged using the TNM method, which focuses on tumor growth and the amount of area affected by the cancer. The 'T' portion lists the size and progression of the primary tumor at the starting of the cancer and can range from T0 to T4. 'N' indicates whether the lymph nodes have been affected and to what extent. It ranges from N0 to N3. Lastly, the 'M' variable displays whether or not the cancer has metastasized and can only be M0 or M1. For the T and N portions, TX or NX may be used if the information needed for the classification is not available.

Using the TNM method, the cancer is then broken down into four stages. Stage I is determined by T1, N0, M0 and has a very localized tumor with no spreading of the cancer detected. This stage is considered the best for surgical intervention and has the highest survival rates. Stage II consists of T2, N0, M0 and while the tumor is localized, it is bigger than in Stage I and there is evidence that the cancer has spread to a small amount of the surrounding area. Surgery is typically still possible but it a bit more difficult at this stage. While the prognosis is still good for a mesothelioma, it's slightly worse than the prognosis of Stage I.

Stage III can be determined by a few different TNM combinations. T1-T2, N1-N2, M0 or T3, N0-N2, M0 are the ranges that indicate an increased progression of the mesothelioma. At the stage, the lymph nodes, lungs, esophagus, diaphragm, and nearby organs may all be affected by the cancer. Stage IV is can also be determined by multiple combinations within the TNM method that include T4, N0-N3, M0, T1-T4, N3, M0, and T1-T4, N0-N3, M1. While the tumor size and lymph node involvement can vary, any metastasis will cause a Stage IV classification. At this last stage, the cancer has spread throughout the body and is affecting multiple organs. For both Stage III and Stage IV, surgery is rarely an option and chemotherapy is often the primary treatment. Since Stage IV makes all treatments difficult, the medical focus is typically on palliative care.
